PROCEDURE & FORMS

1. Complete the PLAS APPLICATION FOR ADMISSIONS. 
    

2. Complete the PLAS ENROLLMENT CONTRACT. 
    

3. Make sure you sign and date the both documents where applicable. 
    

4. Mail or bring in your completed Application and Enrollment Contract to:

5. If you have difficulty or need help with either of these documents, please call 334-566-2023.

NOTE: The Foundation Fee and the Registration Fee must be paid prior to board approval.

THE BOARD WILL REVIEW EACH APPLICATION RECEIVED. PLAS WILL CONTACT YOU ONCE THE BOARD OF TRUSTEES HAS MADE THEIR DECISION.
